• W-4s and/or W-9s

• 1099 forms (if any)

• Specified information that reflects your business:

  • Hourly rates and/or salary and/or temporary employee pay rate

  • Sick days and/or holidays

  • Overtime and/or bonuses

  • Absent pay and/or PTO (paid time off)
